The IHWA was established to identify and assess Humana Medicare Advantage members who are at risk of excessive utilization of future health related services and to supplement Humana’s Health Assessment Program.

Responsibilities

The Care Management Support Professional contributes to administration of the clinical department of YHA. Provides non-clinical support to the assessment and evaluation of members’ needs and requirements to achieve and/or maintain optimal wellness, assist in process documentation and improvement. The Care Management Support Professional routinely collaborates with the clinical department and leadership supporting non-clinical functions including reporting, creating and updating department policies and procedures, and updating and creating Visio documents.

Required Qualifications

1-2 years of professional work experience in an administrative role
Experience creating and maintaining reports
Proficiency in Microsoft Suite: Visio, PowerPoint, Word, Excel, Outlook
Decision making skills regarding own work approach/priorities, and work assignments, standards and resources
Must be passionate about contributing to an organization focused on continuously improving consumer experiences
Must have a separate room with a locked door that can be used as a home-office to ensure you and your members have absolute and continuous privacy while you work.
Must have accessibility to high speed DSL or cable modem for a home office (Satellite internet service is NOT allowed for this role); and recommended speed for optimal performance from Humana systems is 10M x 1M

Preferred Qualifications

Bachelor’s degree
Administrative experience in the healthcare field

Work at Home – desired location in EST

Schedule: Monday-Friday 8AM-4:30PM EST or 9AM-5:30PM EST
